Process Of Admission
In the time of COVID-19, it is important for Camillus MedHaven to maintain proper precautions when admitting new residents. If you are interested in admitting a loved one as a new resident, please follow the admission process below:
Who May Be Admitted?
An elderly, a retiree can be admitted at Camillus MedHaven Nursing Home. He or She may have the following medical conditions: Alzheimer’s or other forms of dementia, recovering from Stroke, Parkinson’s, Depression, recovering from an illness, or in need of palliative care. Any person with no medical conditions that require specialized nursing care is admitted at Camillus MedHaven Nursing Home
Admission Process Steps

A member of the family of the person to be admitted is asked to visit Camillus MedHaven to conduct an ocular visit of Camillus MedHaven and fill out an application form.
Online or over the phone inquiry is allowed and forms may be emailed as well (Application Form, Immediate Family/Sponsor’s Personal Profile, and SPA Form).

Submit a recorded video showing the actual health status of the person to be admitted, Medical Abstract if recently hospitalized, and COVID-19 Test with Negative Results (if available)

Clearance for admission will be secured from the Doctor of Camillus MedHaven.
For those who do not have a Medical Asbract, a Pre-Admission Screening will be done on the day of transfer at St. Camillus Medical Center located in Santolan, Pasig City.

The family will provide the payment and sign the Memorandum of Agreement and Special Power of Attorney at Camillus MedHaven or off-site via scan copy.

The elderly or retired person who is admitted would undergo minimum of 5 days Mandatory Quarantine is an isolation room of the facility prior to full services to commence and transfer and transfer to the room of choice and family members will undertake orientation with members of the Care Team.
